How to Get the Most out of Your Primary Care Visits

Your primary care provider (PCP) is your advocate, guide, and partner in long-term health. We make the most out of visit with you. To maximize the value of your visit, it's important to prepare and effectively communicate. The more prepared you are, the better we can care for you.

Step 1: Preparation is Key (Before You Arrive)

1. Define Your "Why" and Prioritize

Before you call to book, know the one or two most important things you need to discuss. Don’t schedule a physical and then try to squeeze in several new, complex, or chronic issues.

  • For Acute Issues: Focus on the main symptom.
  • For Chronic Issues: Choose one or two specific areas for review (e.g., "I want to focus on my blood pressure management today," or "I want to discuss options for my chronic knee pain").
2. Prepare Your Symptom Log

When describing a symptom (like pain or fatigue), concrete details are far more helpful than general feelings. Write down the answers to these questions:

  • Onset: When did it start?
  • Location: Where exactly does it hurt?
  • Frequency: Does it come and go, or is it constant?
  • Severity: Rate it on a scale of 1 to 10.
  • What makes it better or worse?
3. Bring the Full Medication List (The Most Important Step!)

Bring a complete and up-to-date list of everything you take, including:

  • Prescription medications (name, dosage, frequency).
  • Over-the-counter medications (e.g., pain relievers, cold meds).
  • Vitamins, herbs, and all supplements.
  • Tip: Take a photo of the original bottles and keep it on your phone for easy access.

Step 2: Communication and Clarity (During the Visit)

When you are face-to-face with your provider, use the time efficiently to establish a clear plan.

1. Be Honest—Even If It's Hard

It is essential to be completely transparent about habits, symptoms, and medication adherence. We are here to help, not to judge. Complete honesty leads to the most accurate diagnosis and treatment plan.

2. Stick to the Agenda

Start the visit by briefly stating your top one or two concerns. For example: "I really need to talk about this persistent headache and getting a refill on my blood pressure medication." This ensures the most critical issues are addressed first.

3. Ask "Why" and "What If"

Don't be afraid to ask for clarification. If your provider recommends a new medication or test, ask:

  • Why is this test/medication necessary?
  • What are the potential side effects or risks?
  • How will this change my daily routine?
  • What if I don't follow this treatment plan?
4. Take Notes

Between discussing symptoms, reviewing labs, and setting a treatment plan, there is a lot of information shared. Bring a small notebook and pen to jot down key points, or ask if you can use your phone to record notes. This prevents "I forgot what the doctor said" syndrome later!

Step 3: Follow-Up for Success (After the Visit)

The health journey doesn't end when you walk out the door. The time immediately following your visit is critical for executing the plan.

1. Confirm the Next Steps

Before leaving the office, confirm the following with the staff:

  • Do I need to schedule a follow-up appointment? If so, when?
  • WIll any referrals sent to specialists?
  • Do I need bloodwork?
2. Follow the Treatment Plan

Whether it's a new medication, dietary changes, or referrals, commit to the plan as discussed. If you encounter side effects or have trouble accessing your prescribed care, contact us right away. Do not stop treatment without consulting your provider.

3. Utilize the Patient Portal

Use our online portal to communicate non-urgent questions, request refills, and review your notes and lab results. This allows us to handle your request efficiently without needing a full appointment slot.
